integral role in the home health care process. To stress the importance
of health care education and strive to help patients and families
thoroughly understand their health situation. Guided by the love of
Jesus Christ, Blessed Trinity Home Health Care is committed to meeting
the home health needs of the people which it serves.
Kansas Service Area:
Wyandotte, Leavenworth, Atchison, Johnson,
Jefferson, Douglas & the northern part of Miami county.
Missouri Service Area:
Clay, Platte, Jackson County.
Blessed Trinity Home Health Care is a free standing, nurse owned,
home health agency based in Wyandotte County with a branch office in
Leavenworth County. Entering our fifth year of serving the
community, BTHH was designated as an “Elite Agency” in 2006 and 2007
by the Centers for Medicare and Medicaid. “Elite Agencies” are
considered to be the top 10% (hospital affiliated and free standing)
across the nation among Medicare certified agencies.
We pride ourselves on the compassionate, patient-centered care
that our skilled clinicians provide.
At Blessed Trinity we have structured our clinical staff into
clinical
specialty teams to allow us to align our staff’s strengths and
experience to better serve our patients and their families.
Patient and family education and support are essential to the
success of our care. The role of home health is to provide short
term acute care support for patients who need care following an
illness, injury or progression of a disease process that renders the
patient
homebound and in need of the
skilled assessment and intervention of a nurse or therapist.
Home Health Criteria for Admission
- Physician Orders – The physician prescribes specific orders
for the home health plan of care of each individual patient.
- Homebound – An individual is unable to leave
their home unassisted, and/or leaving takes considerable and
taxing effort.
- Intermittent Skilled Need – An individual
requires short-term, intermittent care that is provided by at
least one of the following: a licensed nurse, physical
therapist, speech therapist, or has a continuing need for
occupational therapy.
Home Health services are generally covered by
Medicare, Medicaid and most private insurance when the criteria for
admission applies.
