Skilled Nursing Services – Patient Care Coordinators lead teams of nurses with specialized skills, training and experience to meet the individual needs of patients receiving care in the home.




Rehabilitation Services – Our multidisciplinary team of therapists focus individualized treatment on facilitating a safe recovery of function and mobility at home. Patients often transition from home health physical, occupational and/or speech therapy to an outpatient setting for further rehabilitation.

Homemaker Services – Services charged hourly and paid via monthly invoice by patient or family member. A minimum hourly service contract per week is required. Acceptance is based on staff availability. Contact our office at 913-721-9856 for current schedule availability and rates. Waiting lists may be utilized if staffing is limited.

Home Health Aide Services – All aides hold certifications for both Home Health Aide (HHA) and Certified Nurses Assistant (CNA).
  • Covered by some insurance for short term bathing assistance when a patient is expected to regain independence with bathing
  • Collaborates often with Occupational Therapy
  • Must have another skilled service in place simultaneously (SN, PT, OT)
  • Service must focus on bathing patient

In-Home Tele-Monitoring – can be utilized to monitor vital signs daily and track patient response to treatment and compliance for cardio/pulmonary conditions. Monitors are fully programmable and parameters can be individualized by patient ability, need and/or diagnosis.

Medical Social Worker Consultation – Services provided for our patients when ordered by a physician as part of a current home health plan of care.
  • Immediate social/economic/safety concerns
  • Exploration of community resources for specific needs
  • Placement facilitation for dependent patients
