individualized treatment on facilitating a safe recovery of function and mobility at home. Patients often transition from home health physical, occupational and/or speech therapy to an outpatient setting for further rehabilitation.

Homemaker Services – Services charged
hourly and paid via monthly invoice by patient or family member. A
minimum hourly service contract per week is required. Acceptance is
based on staff availability. Contact our office at

for current schedule availability and rates. Waiting lists may be
utilized if staffing is limited.

Home Health Aide Services – All aides hold
certifications for both Home Health Aide (HHA) and Certified Nurses
Assistant (CNA).
- Covered by some insurance for short term bathing assistance when
a patient is expected to regain independence with bathing
- Collaborates often with Occupational Therapy
- Must have another skilled service in place simultaneously (SN,
PT, OT)
- Service must focus on bathing patient

In-Home Tele-Monitoring – can be utilized
to monitor vital signs daily and track patient response to treatment
and compliance for cardio/pulmonary conditions. Monitors are fully
programmable and parameters can be individualized by patient
ability, need and/or diagnosis.

Medical Social Worker Consultation –

- Placement facilitation for dependent patients
